When you call, our team will work with you to identify your eligibility for free or low-cost medical services and/or insurance programs. Our teams will then make an appointment for you to meet with the appropriate health care provider depending on your individual health care needs.

If you have health insurance

We make it easy for you to become a patient. Call Patient Services and have your insurance information available to make an appointment at the health center of your choice. We accept most private insurance plans, including Medi-Cal, Medicare, and Healthy Families.

If you are uninsured

We believe in health care for all. Call Patient Services to make a pre-registration appointment. We will determine your eligibility for our various programs.

Preparing for your visit.

Before a visit you might be nervous, impatient, or even scared. Being prepared can help you get your questions answered and concerns addressed. Here are some things you can bring to make the most of your visit:

  • List of your concerns, any allergies and all the medicines, herbs or vitamins you take
  • A description of symptoms – when they started, what makes them better, what makes them worse
  • A list of any physical or mental health changes since your last visit
  • A trusted friend or relative
  • A notepad

Make sure you understand your diagnosis and treatment. Ask your health care provider to write down his or her instructions for you.
