COVID-19 Information

The health and safety of our patients and staff are our top priority. Depending on Community Transmission Rates, we may limit the number of support people allowed at in-person visits to reduce the risk to our patients and staff.

COVID-19 Vaccine

COVID-19 vaccines are effective at protecting you from getting seriously ill, being hospitalized, and dying due to a Covid-19 infection. As with other vaccine-preventable diseases, you are protected best from COVID-19 when you stay up to date with the recommended vaccinations and boosters. They also offer added protection after recovering from a COVID-19 infection. 
 
COVID-19 vaccination is recommended for all people ages 6 months and older. We offer the Moderna primary vaccine and booster and may have limited Pfizer, by request. Depending on your age, type of vaccine received, and certain health characteristics, people will need 3-4 vaccines for optimum protection.

Vaccines are available to LifeLong patients only.

Vaccines for Minors

All minors under the age of 18 will need parent/guardian consent to get vaccinated. Consent can be given in TWO ways:

  • Written: Print the form below and bring it signed to your appointment, or parent/guardian can sign at appointment if accompanying minor.
  • Verbal: Parent/guardian must be available at time of appointment to give consent over the phone.

If you are a LifeLong patient and have tested positive for COVID-19, you may be eligible for treatment.

COVID-19 Testing

Limited appointments are available at our William Jenkins Health Center (Richmond) And East Oakland Health Centers.

All of LifeLong Medical Care's COVID-19 testing sites offer both drive-thru testing (for people in cars) and walk-up testing (for people without cars).
