COVID-19 Information

The health and safety of our patients and staff are our top priority. Covid-19 Community Transmission Rates have been High in Alameda, Contra Costa, and Marin counties and continue to rise. To reduce the risk to our patients and staff during in-person appointments, beginning May 23, 2022:

Adult patients must attend in-person appointments without a support person with limited exceptions (cognitive impairment, conservatorship, case management, or signed Release of Information form on file.)

Pregnant patients can bring one support person 

Only one parent or guardian can accompany a patient under 18 years old  

One extra support person can accompany a newborn (under 2 months old)

COVID-19 Vaccine

Vaccines are only available to LifeLong patients.

See below for vaccine eligibility. Visit the CDC website for more information on boosters.

Availability of vaccines

  • Ages 6 months through 4 years: Moderna vaccine only
  • 5 year olds: Moderna or Pfizer available
  • Ages 6 through 17 years old: Pfizer vaccine only
  • Ages 18+: Moderna vaccine (Pfizer by request)

Primary series

  • 1st dose: LifeLong patients ages 6 months+
  • 2nd dose: LifeLong patients ages 6 months+
  • 3rd dose: LifeLong patients ages 12+ who are immunocompromised

Boosters

  • 1st booster: ages 5 and up (must have completed primary series with Pfizer/Moderna at least 5 months ago, or received single dose of Johnson & Johnson at least 2 months ago)
  • 2nd booster: received first booster at least 4 months ago AND meets one of the below requirements:
    • Everyone ages 50+
    • Immunocompromised, ages 12+
    • Received Johnson & Johnson as 1st dose

Vaccines for Minors

All minors under the age of 18 will need parent/guardian consent to get vaccinated. Consent can be given in TWO ways:

  • Written: Print the form below and bring it signed to your appointment, or parent/guardian can sign at appointment if accompanying minor.
  • Verbal: Parent/guardian must be available at time of appointment to give consent over the phone.

We offer COVID-19 vaccines at four of our LifeLong Health Centers. The clinics are held outside, so please dress accordingly.

We do our best to accommodate walk-ups, but your appointment will go faster if you schedule in advance.

COVID-19 testing is available to LifeLong patients.

Limited appointments are available at our William Jenkins Health Center (Richmond) And East Oakland Health Centers.

All of LifeLong Medical Care's COVID-19 testing sites offer both drive-thru testing (for people in cars) and walk-up testing (for people without cars).
