COVID-19 Vaccine

Vaccines are available to LifeLong patients and the community. Free for everyone – Open to the Public – No insurance or documentation needed – Walk-up’s welcome without an appointment.

We offer the Pfizer vaccine at all of our sites. The Johnson & Johnson vaccine is available by request.

Vaccines for Minors

All minors under the age of 18 will need parent/guardian consent to get vaccinated. Consent can be given in TWO ways:

  • Written: Print the below form in advance and bring it signed to your appointment, or parent/guardian can sign at appointment if accompanying minor.
  • Verbal: Parent/guardian must be available at time of appointment to give consent over the phone.

We offer the vaccine at four of our LifeLong Health Centers. The clinics are held outside, so please dress accordingly.

COVID-19 Testing

LifeLong offers COVID-19 testing at outdoor testing sites located in Oakland, Berkeley, and Richmond. Testing is available to LifeLong patients and the community.

Free for everyone – Open to the Public – No insurance or documentation needed – Walk-up’s welcome without an appointment.

All of LifeLong Medical Care's COVID-19 testing sites offer both drive-thru testing (for people in cars) and walk-up testing (for people without cars).

The pandemic isn't over. The virus is still spreading. Getting tested for symptoms, even if you're vaccinated, is an important part of stopping the spread.

Be aware of COVID-19 symptoms, which can appear 2-14 days after exposure:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

When exposed to COVID-19, unvaccinated should get tested

Unvaccinated people who have been in close contact with someone who tested positive for COVID-19 should get tested.

Vaccinated people who do not have symptoms of COVID-19 but have been in close contact with someone with COVID-19, do not need to get tested. Exceptions include those who live or work in congregate settings, such as a dormitory.
