LifeLong Medical Care is committed to providing the best possible patient experience by employing a patient-centered, team-based approach, utilizing emerging technologies, and ensuring a fully trained and passionate staff.

As part of our commitment, we are working towards national recognition for all health centers through HRSA Health Center Profile program. This program provides a roadmap for how to organize care around patients and work in teams to coordinate, track, and improve care.

Patient Voice Collaborative

LifeLong Medical Care’s Patient Voice Collaborative (PVC)  is a diverse group of patients, providers, and staff that meet monthly to discuss ways in which LifeLong can improve the efficiency, quality, and accessibility of care for all patients. As patient members, you will be asked to share insights and information about your experiences so that we can work towards a more patient-centered system of care.

LifeLong Patient Voice Collaborative

Alameda County Fathers Corps

LifeLong is in a partnership with Fathers Corps, a collaborative effort between First 5 Alameda County and Alameda County’s Health Care Services Agency and Social Services Agency, was formed to support fathers to be meaningfully engaged with their children and to advocate for father-friendly services. Their fatherhood work is focused on empowering and supporting:

Fathers, Service providers, Agencies, Systems and the wider community.
