LifeLong Medical Care Telehealth Services allow you to have an audio or video-based appointment with your provider. It makes routine healthcare more convenient and easier for you to access.

LifeLong has a Virtual Care Representative to support you with virtual care which includes things like MyChart and video visit preparation.

What is a telehealth appointment?

A telehealth visit is similar to an in-person wellness appointment with your own doctor. During the visit you’ll talk about your current health concerns and learn what to do.

  • Your visit can take place from a location that is the most convenient for you.
  • Your provider will use a video-based platform to talk with you by computer, tablet or smartphone.
  • Our telemedicine platform is secure and HIPAA compliant.
  • Your photo and voice will not be recorded or stored.
  • Your privacy and rights will be respected and ensured.

Preparing for your telehealth appointment

After scheduling your appointment by phone or MyChart, you will be sent a link with information about your appointment. When it is time for your appointment to begin, you’ll click on this link to be connected with a provider.

If using a smartphone/tablet for your virtual appointment:

  1. Download the MyChart app (MyChart is available for Apple® or Android™) onto your device.
  2. Choose LifeLong Medical Care as the organization.​
  3. Log in with your existing MyChart account or create an account by clicking "Sign up now."
  4. Download the Zoom Cloud Meetings app onto your device and allow it to access your camera and microphone. You do NOT need to create a Zoom account.​
