MyChart is an easy, confidential way to access your care team and medical information online. You can see most of your test results and clinical notes in MyChart as soon as they become available. We’ll contact you about any results that concern us.

Create Your MyChart Account

1. Register for MyChart online –

    • If you received a 10-digit activation code at your last appointment, register online.
    • If you do not have an activation code, you need to do is provide some basic information to register without an activation code.

2. Register at your provider's office

      • At your next appointment, ask to set up a MyChart account and they can help you register.

3. Register by phone

Is there a MyChart App?

MyChart is available for Apple® or Android™ so you can have access to your health care information anywhere you have a smartphone. Once you’ve downloaded the app, choose LifeLong Medical Care as your healthcare organization. Then log in with your username and password.
